PLEASE NOTE

THE NHLPA GOALS & DREAMS FUND IS NOW ACCEPTING NEW APPLICATIONS FOR THE 2007-2008 GIVING CYCLE.

PLEASE COMPLETE THE FOLLOWING FOR YOUR ORGANIZATION TO BE CONSIDERED FOR AN EQUIPMENT GRANT:

• Submit a typed two-page letter explaining the program objectives, history of the program, program budget and information regarding community support the program receives.
• Formal Goals & Dreams application sent to applicant whose program will merit consideration.
• Goals & Dreams staff reviews application.
• Goals & Dreams staff may follow-up with applicant to request more information on program.
• Advisory Board recommends approval, revision, deferral, or rejection.
• Applicant is notified of final decision.

The Goals & Dreams Fund gives prompt consideration to all proposal letters. However, the initial review process often takes four to six weeks to complete. All applicants will be notified of the status of their request.
